Evaluator Consultant Database

Diversity in Health Evaluation Consultants

The Endowment has developed a database to serve as a resource for grantees and other community-based organizations that seek evaluation consultants to assist in their efforts to document grant-making impact and facilitate more effective program implementation.

In you are an evaluator interested in joining the database, The Endowment is interested in learning more about consultants with the presence and capacity to work in California, and who have familiarity with California's diverse populations. Please consider these priorities before submitting your name to the database.

If you are interested in being added to the database, please take a moment to complete the profile. The database allows visitors to submit their information as individual consultants, but if you would like to register as an organization only, please fill the First Name and Last Name fields with "N/A."

If you are an evaluator who is part of a larger organization, we encourage you to consider submitting both an individual and organizational summary of expertise to increase your chances of coming up in a future database search.

Please fill out the following information and select the “submit” button at the end of the form when completed. Fields marked with an asterisk (*) are required fields. While not all fields are required to submit this form, your likelihood of being a part of a potential consultant search is enhanced by completing all fields.

If you have any questions or problems filling out this form, please contact Betty Holt by phone at (213) 928-8672 or by email at evaldatabase@calendow.org.
